New patient consultation (60 minutes): $350
Follow-up visit (30 minutes): $150
Telehealth visits: same prices as in person
What's included: everything your care from that visit requires. That covers ordering labs and imaging, prior authorizations, prescriptions, referrals, and coordinating with your other doctors. No surprise charges for the work that follows your visit.
Not included:
Disability, FMLA, legal, or insurance paperwork (available for an additional fee; just ask)
Tests done by outside labs and imaging centers, which bill you directly. We'll help you find affordable options.
We don't bill insurance. On request, we can give you a detailed receipt (superbill) to submit to your insurer for possible out-of-network reimbursement.
Procedures
Botox for chronic migraine, spasticity, cervical dystonia: $150 procedure fee, does not include medication
Nerve blocks: $150
Trigger point injections: $150
EMG/nerve conduction studies: $350, performed and interpreted by Dr. Stephenson
Good Faith Estimate notice You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance, or who aren't using insurance, an estimate of the bill for medical items and services. You can ask for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
